Negative vs positive canthal tilt
Both describe the same thing: the angle of the line between the inner and outer corner of your eye. Positive means the outer corner sits higher, negative means it sits lower, neutral means they are close to level. The bands are reporting conventions, not natural categories, and the same face can land in two different ones depending on how the photo was taken.
This guide is for entertainment and styling reference. A photo estimate is not a diagnosis and not medical advice.
The difference in one line
Draw a line from the inner corner of your eye to the outer corner, then compare it to the horizontal. Positive: the outer corner is higher. Negative: the outer corner is lower. Neutral: they are close to level. That is the entire distinction — one line, one angle.
You will also see it written as upward and downward tilt, or as high and low canthal tilt. They refer to the same measurement.
The three readings side by side
| Reading | Where the outer corner sits | Also called |
|---|---|---|
| Negative | Lower than the inner corner. Below 0° on this site | Downward tilt, low canthal tilt, downturned eyes |
| Neutral | Close to level with the inner corner. 0–2° here | Level tilt, straight eyes |
| Positive | Higher than the inner corner. 2° and above here | Upward tilt, high canthal tilt, upturned eyes |
| Near a boundary | Within a degree either side of a cutoff | Read it as near the edge, not as firmly one band |
| Different in each eye | One eye reads higher than the other | Common; may come from head position or landmark placement |
| Different between photos | Same face, two readings | Camera position and head rotation change what a photo reports |
Where the boundaries sit, and why they move
On this site the reporting bands are: below 0° negative, 0–2° neutral, 2° and above positive. Other sources draw them elsewhere, because there is no agreed clinical cutoff — these are conventions for reporting a photo estimate, not thresholds that mean anything on their own.
Published averages vary by population and by measurement method, and studies are not directly comparable when landmarks, head position or reference planes differ. A reading close to a boundary should be read as “near the edge” rather than as belonging firmly to one side.
Why the same face measures differently
This is the part most comparisons leave out. Camera position and head rotation change the angle a photo reports, and the size and direction of that change depend on the setup. A shot taken from below, or with the chin lifted, reports a lower tilt than a level, front-on shot of the same face.
Your two eyes may also differ from each other. Small differences in a photo can come from head position or from where the landmarks were placed rather than from anatomy.
So a face that reads negative in one selfie and neutral in another has not changed. The photograph did. Take a level, front-on shot before concluding anything — the canthal tilt test reports each eye in degrees so you can see how close to a boundary you actually are.
What each reading does not tell you
- It is not a ranking. Positive is not a better result and negative is not a worse one. It is a description of where two points sit.
- It is not a health finding. A tilt in any band on its own does not establish a medical problem, and a photo measurement cannot assess eye health.
- It does not predict how a face reads. Brow shape, eye shape, lid exposure and expression all affect the impression far more than the number does.
- It is not fixed for life. Age-related changes in eyelid support can affect lid position and contour, though the amount and direction vary between individuals.
Which pages to read next
Each band has its own page with what the reading means and where the figures come from: negative canthal tilt, neutral canthal tilt and positive canthal tilt.
If what you actually want to know is what can be changed, what moves a canthal tilt and what does not covers brows, liner, camera angle and what the surgical terms mean.
Seek medical assessment for a new change in eyelid position or difficulty closing the eye. Sudden eyelid drooping, eye pain or changes in vision need urgent assessment.
Questions people ask
What is the difference between negative and positive canthal tilt?
Positive means the outer corner of the eye sits higher than the inner corner; negative means it sits lower. Neutral means they are close to level. All three describe the same measurement — the angle of the line between the two corners against the horizontal.
Is positive canthal tilt better than negative?
Neither is better. The bands describe where two points sit relative to each other, not how a face looks or how healthy it is. Plenty of faces people find striking measure negative.
What is the difference between low and high canthal tilt?
They are alternative names for the same thing. Low canthal tilt means the outer corner sits lower — the same as negative. High canthal tilt means it sits higher — the same as positive.
Can I be neutral in one photo and negative in another?
Yes, and it is common near a boundary. Camera position and head rotation change the angle a photo reports without anything about your face changing. Take a level, front-on shot to get a more consistent reading.
Where exactly is the line between the bands?
There is no agreed clinical cutoff. This site reports below 0° as negative, 0–2° as neutral and 2° and above as positive, and other sources draw the lines elsewhere. Treat a reading near a boundary as near the edge, not as firmly one category.
